MEDICARE SELF ENROLLMENT

1. Click Link Below to Enter

 2. Enter your Zip Code

3. Click the Blue View Plans Tab

4. Enter the Effective Date (First Day of the next Month)

5. To find your provider, click the Blue Get Started Tab, then proceed to View Plans.

6. Under Filters, find the company of your choice, and your Network Type, (HMO or LPPO). 

7. Find your plan, and view the plan details, if you have any questions contact Agent Joe Gray Sr.
(The Summary of Benefits can be found at the bottom of the view the plan detail page)    

8. Click Enroll, fill out your Name, Email Address, Phone, and 'Continue'.

You now have four more steps to complete your enrollment, 'Review', and 'Submit'.
